Macmillan 1-1 Support Service

Macmillan 1-1 Support services
The Macmillan 1-1 Support Service Team offer community support to patients and loved ones who are affected by cancer, the support extends from diagnosis, through to treatment, whilst in recovery and then living with and beyond. The Macmillan’s 1-1 Support Services are available to anyone over the age of 18 who lives in Oldham, has an Oldham GP or pays Council Tax to Oldham. Macmillan accept self-referrals and referrals from family members, clinicians and GP’s. Upon referral a holistic needs assessment is completed to ensure the patient is able to access all the appropriate services.

Macmillan 1-1 Support Service Team
The Macmillan 1-1 Team, made up of both nursing and administrative staff, are benefitting from being based at the Hospice as they have the opportunity to host some of their health and well-being events and coffee mornings in-house. The team also now have an added option, apart from offering a home visit or telephone consultation, to invite patients into the Hospice for a face-to-face appointment to identify their current concerns and plan their ongoing personalised care and support.

Macmillan have been so helpful and supportive.
Having the team at the Hospice has meant it has been easier for me to access their services and to then be referred to the Wellbeing Centre as they are working so closely together, it’s the perfect partnership!
