Our Hospice at Home service provides patients and their families with the appropriate level of additional care to ensure the patient has the support they need to remain in their own home, if that is their preferred place of care, and to prevent unnecessary hospital admissions.

Our Hospice at Home Team
Our Nursing Team consists of experienced hospice nurses who visit patients in the last weeks / months of their lives, providing a flexible and responsive nursing service in the patient’s own home. The Hospice at Home service is available daily from 7am-9pm and is designed to work alongside, and in addition to, services provided by the Community Nursing Team, Home Care providers and Community Specialist Palliative Care Nurses.

Our patients
Our team visit patients and families to provide patient care and family support based on individual needs. The timing, length and frequency of visits is flexible and is determined by the needs of the patient. When a patient is accepted into the Hospice at Home service, family members will have access to the team delivering care by calling the Hospice at Home mobile telephone number. This will be provided by the team when they visit.
How to be referred?
Our Hospice at Home service is available to Oldham residents who are registered with an Oldham GP. For referrals, please contact our Community Services Manager on 0161 624 2727.
When the Hospice at Home Team arrived they were absolutely amazing, they cared for Karen and everyone else. My sister was their priority, and they couldn’t have done enough for her.

The advice line is available seven days a week, 365 days a year and is manned by trained and experienced nursing staff.
