Dr Kershaw’s Hospice works closely with other health care professionals to ensure we have the full picture and understanding of each patient. This enables us to provide the best possible care to each person, caring for every patient the way they choose to be cared for at the end of their lives.

Referrals & admissions

Referrals for Hospice admission are accepted from a variety of sources including GPs, hospital consultants, community nurses, Specialist Palliative Care Nurses, hospital and community Allied Health Professions (AHPs). In general, referrals are accepted for patients who are residents within the boundary of Oldham and its surrounding districts although a small percentage of admissions are from outside the immediate area. The population served by the Hospice is around 242,000 people.

The Hospice operates a planned admission policy. The clinical palliative care Multi-Disciplinary Team (MDT) have input into the planning of patient admissions and assist in assessing priorities.
Patients may be admitted at any stage during their illness for appropriate treatment and support. Admission may be for complex symptom control, or care at the end-of-life.

Inpatient Unit referral

We admit patients to our Inpatient Unit for care in the final days of life, and for complex pain and symptom control challenges.

Hospice at Home referral

We care for patients in their homes in the final days of life, generally limited to a 2-week duration so that we can remain responsive to urgent palliative care needs.

Our Hospital Specialist Palliative Care Team works closely with Dr Kershaw’s Hospice.

This collaborative working allows us to provide compassionate, patient-centred care that respects each individual’s needs, fostering an environment where patients feel supported, valued and cared for throughout their journey.
